Dural tears, involving the accidental tearing of the dura mater, are often more prevalent than widely acknowledged. They can lead to cerebrospinal fluid (CSF) leaks, posing significant challenges in post-operative care, prolonged hospitalization and increased healthcare costs.
In a report from 2022⁽¹⁾, Danish researchers investigated the incidence and risk factors of incidental dural tear in patients undergoing lumbar spine surgery. Data was extracted from the Danish national database for spine surgery (DaneSpine), with a total of 1,139 surgical procedures for lumbar spinal stenosis and lumbar herniated disc included.
A wide variety of surgical approaches from limited decompression to fusion plus fixation were included in the study. Among these 1,139 lumbar surgeries for degenerative pathology an overall incidence of 10.4% in dural tears was noted. The occurrence of dural tears were higher in surgery for lumbar stenosis than for lumbar herniation and reoperations also carried a higher risk of dural tears.
These data are in line with other international studies, reporting incidence of incidental durotomies in spinal surgery of between 1.6% to 16%⁽²⁾. Dural tears or incidental durotomies carries the risk of higher healthcare costs and poorer patient outcomes⁽³, ⁴⁾.
Understanding the factors leading to dural tears is crucial. They range from surgical complications like adhesions or direct trauma to patient-specific factors like age and previous surgical history. But even more important is to take every precaution possible to avoid these complications.
